Open hours：9:00am-4:00pm

  1. Please contact by phone before visiting medical institutions.
  2. When visiting medical institutions, please bring your "My Number" card resistered as your health  insurance card or other proof of eligibility such as the Shikaku Kakuninsho (資格確認書, Health Insurance Eligibility Certificate).
