A pre-vaccination questionnaire is necessary to get vaccinated.
● If you wish to receive vaccinations including vaccinations for children, HPV vaccinations, and vaccinations for the elderly (i.e. influenza and pneumococcal vaccinations) after moving to Tsukuba City from another municipality, you need to receive pre-vaccination questionnaire from Tsukuba City. Please note that you cannot use pre-vaccination questionnaires issued by your previous municipal office.
You may ask for questionnaires at the Health Promotion Division (counter #㉔) / the Vaccination and Infection Control Measures Office on the 1st floor of Tsukuba City Hall or your nearest health center.
[What to bring]
· Maternal and Child Health Handbook (Not required for vaccinations for the elderly)
· Pre-vaccination questionnaires (original) issued by your previous municipal office
● If you wish to get a vaccination at a healthcare facility other than the affiliated healthcare facilities of Tsukuba City *1, you need to apply for it at least two weeks in advance as a general rule.
Please note that without the advance application, you will have to pay the full cost of the vaccination.
When receiving routine vaccinations at a facility other than a cooperating medical institution
Vaccinations are offered at the following healthcare facilities:
① Healthcare facilities which are members of the Tsukuba Medical Association
and affiliated to the city in providing vaccinations.
② Healthcare facilities which are members of the Ibaraki Medical Association
③ Healthcare facilities other than the above (Advance application required)

New residents who are pregnant or have a child under 1

If you are going to have maternity checkups, infant health checkups and/or a newborn hearing test:
● You cannot use coupons issued by your previous municipal office.
You need to ask Tsukuba City to issue coupons for checkups/test.
You may ask for the coupons at the Health Promotion Division (counter #㉔) / the Vaccination and Infection Control Measures Office / the Child Welfare and Parental Support Center on the 1st floor of Tsukuba City Hall or your nearest health center.

· Maternal and Child Health Handbook
· Vaccination coupons (original) issued by your previous municipal office
Parents of pregnant women, mothers, and infants who have moved into the country.
If you are going to have the above mentioned health services at a healthcare facility outside Ibaraki Prefecture:
● You need to apply at least one month in advance as a general rule to have maternal checkups, a newborn hearing test, or infant general health checkups outside Ibaraki Prefecture.
- If you are going to be late in submitting the advance application, please call us.
- The cost of all infant general health checkups at a healthcare facility outside Ibaraki Prefecture will be reimbursed.
- Please note that without the advance application, you will have to pay the full cost.
The Tsukuba City Child Welfare and Parental Support Center will open.
The Tsukuba City Child Welfare and Parental Support Center is a consultation and support center for pregnant and postnatal women, expectant parents, and families with children who may have problems or concerns. The center also issues Maternal and Child Health Handbooks.
