One month old baby checkup outside Ibaraki Prefecture

You must submit an application form to have a one month old baby checkup outside Ibaraki Prefecture to Sakura, Yatabe or Oho Health Center one month prior to your baby's checkup as a general rule. The form is called Application Form for Out-of-the-prefecture General Pregnancy Health Checkup, Maternity Health Checkup, Newborn Hearing Test, and One Month Old Baby Checkup (県外妊婦一般健康診査・産婦健康診査・新生児聴覚検査・1か月児健康診査受診申請書). The form is downloadable by scanning two dimensional code on the top right.

Important points to note

•If you move out of Tsukuba City by the day of your baby's checkup, your baby will not be eligible for the subsidy. You need to inquire at the municipal office where you are registered as a resident.

•The coupon will be valid up to one day before your baby turns two months old if there are compelling reasons.

•Please refer to the official website of Tsukuba City (scan the two dimensional code on the top right) for healthcare facilities which offer health checkups for one month old babies.
