Booking a maternity consultation appointment (Tsukuba City)
If you are registered as a resident of Tsukuba City and your pregnancy is confirmed at a healthcare facility, you are required to pre-register your pregnancy, and then to book an appointment to officially register (and have a maternity consultation).

Procedures
1.Download the app and sign up
Scan the two-dimensional code on the right to download the Tsukukko! Suku-suku App and sign up your basic information.
①Link your personal account or register E-mail address with the app!
Select your ID or E-mail address.(*)
②Register your profile!
By registering the postal code,childcare information for your local area weill be delivered.
③Register your child's information!
By registering your fetus or child's profile,information matching the age will be received.
2.Pre-register your pregnancy
You may pre-register your pregnancy on the app: Go to the option Local Childcare Information > Childcare Information in Tsukuba City > Advance application for pregnancy notification form /maternal and child health handbook(R6.4~) .
If you have not signed up as a user of Ibaraki Online Application and Registration Services:
You can pre-register your pregnancy without signing up as a user.
If you have signed up as a user of Ibaraki Online Application and Registration Services:
Please be sure to log in the services' website and pre-register your pregnancy. If you are unable to complete the procedures after logging in, keep yourself logged in and access the services' website on the app again.
3.Book an appointment for a maternity consultation
You may book an appointment on the app: Go to the option Local Childcare Information > Tsukuba Online Booking > Select a place where you would like to register your pregnancy > Fill in the required fields, and click Book Online.
You will receive an email soon after. Please complete booking by clicking the link in the email!
Local childcare information>Tsukuba Online Booking>Make a reservation online>Fill in the required fields>Confirm your reservation on the screen.
What to bring with you to register your pregnancy:
- "My Number" card (If you do not have one, bring an ID such as residence card and your driver's license.)
- Pregnancy Confirmation Form (which is issued by a healthcare facility and indicates the name of the facility and your due date.)
- Letter of authorization (Required only if you asked someone other than your household members to register your pregnancy for you.)
Points to note:
- If you are going to register your pregnancy at one of Tsukuba City Hall's branch offices (Tsukuba, Toyosato, Kukizaki or Tsukuba Station), you do not have to pre-register or book an appointment to officially register your pregnancy. However, you are required to have a maternity consultation at the Child Welfare and Parental Support Center or one of health centers (Sakura, Yatabe or Oho) at a later date. Your local health center will contact you to schedule the date.
- Your official pregnancy registration date will be the day you receive a Maternal and Child Health Handbook.
- If you have asked someone else to visit the City Hall or branch office to register your pregnancy for you, you are required to have a maternity consultation at a later date.
- Please be sure to bring the Pregnancy Confirmation Form (this form) with you to your maternity consultation.
- Details of how to apply for the Childbirth Support Payment will be provided at your maternity consultation.
