How to request a COVID-19 vaccination voucher after moving to Tsukuba City

Note: If you already have a Pre-vaccination Screening Questionnaire (yoshin-hyo) for 3rd or 4th dose, a voucher is printed on the right top.

Application is required to issue a vaccination voucher.

If you have moved to Tsukuba City after April 2021 and wish to get vaccinated against COVID-19, you need to apply for a vaccination voucher.

How and what you need to apply for a vaccination voucher

Over the phone application

In-person application

Where to apply	COVID-19 Vaccine Distribution Office located on the 1st floor of the Commuty Builiding, Tsukuba City Hall (1-1-1 Kenkyu-Gakuen, Tsukuba) Open on weekdays from 8: 30 a.m. to 5:15 p.m.

Required document: No documents are required*.

*1: You need to provide a paper-based document showing your vaccination record such as the Certificate of Vaccination for COVID-19, COVID-19 Vaccination Certificate and COVID-19 Vaccination Record, if your record is not reflected in the Vaccine Record System (VRS) properly. *2: If you were vaccinated outside Japan, you need to provide a document showing your vaccination record issued from the country in which you were vaccinated. *3: If you have not been vaccinated yet or if you received a vaccine which is not approved in Japan, please let us know so that we can issue a first dose vaccination voucher.

Online application

Required document: You need to provide a document showing the record of your previous dose or doses (such as the Certificate of Vaccination for COVID-19, COVID-19 Vaccination Certificate and COVID-19 Vaccination Record).

Note: You will not be able to apply online if you cannot provide a document showing the record of your previous dose or doses.

Postal application

Required documents: Application form (downloadable below) and a document showing the record of your previous dose or doses (such as the Certificate of Vaccination for COVID-19, COVID-19 Vaccination Certificate and COVID-19 Vaccination Record).

Note 1: If you are not able to send a document showing your previous dose or doses, you cannot apply by post. Please apply for a vaccination voucher over the phone or in person.
Note 2: If you have not been vaccinated yet or if you received a vaccine which is not approved in Japan, please send an application form only (a document of vaccination record is not required).

Sending of a vaccination voucher For 1st or 2nd dose vaccination: A vaccination voucher will be sent to you approximately 10 days after you applied. (Please note that a vaccination voucher cannot be issued on the spot.) For 3rd or 4th dose vaccination: Please refer to the schedule for sending vaccination vouchers for 3rd or 4th dose vaccination. If the schedule date has already passed, your voucher will be sent to you approximately 10 days after you applied for it.
