Change in the format of Prevaccination Screening Questionnaire for COVID-19 vaccine (1st and 2nd doses)

The format of Prevaccination Screening Questionnaire for COVID-19 vaccination was changed on December 1, 2021.

 

Therefore, if you are receiving your first or second dose of the COVID-19 vaccine on or after December 1, please download a new questionnaire from the following link and bring it to a vaccination site.

If you bring the old questionnaire, you will be asked to rewrite it in the new questionnaire at the vaccination site.
