Vaccination Certificate of COVID-19 (“Vaccine Passport”)

Vaccination Certificate of COVID-19 (“Vaccine Passport”) certifies that you were vaccinated against COVID-19.

Please also note that if you would like to issue a Vaccine Passport in Tsukuba City:
・You must be vaccinated using vaccination vouchers issued by Tsukuba City.
・If you moved out of Tsukuba City, your “Vaccination Passport” can be issued in Tsukuba City as long as you were registered as a resident and vaccinated in the City in the past.

That means if you had your first and second doses in different municipalities because you moved houses or other reasons, you need to request both municipalities to issue a vaccine passport.

Digital Vaccination Certificate

The Japanese Government has launched digital vaccination certificate app on 20th December, 2021 on App Store and Google Play. You can now apply for a vaccination certificate with 2D code on the app, and obtain and display the certificate your smartphone.

You are required to verify your identity using your “My Number Card” to apply for a certificate on your smartphone.

What you need to use the Digital Vaccination Certificate

-Smartphone (NFC Type B compatible devices)
Minimum requirement of iOS 13.7 or Android OS 8.0

-My Number Card
You will need the four-digit PIN you set up when you received your card.

-Passport (for those who are applying for the international travel version)

Note: If you forget your four-digit PIN, you will need to reset it at the Citizen Services Division at Tsukuba City Hall or its branch office.

Persons who cannot apply for digital vaccination certificate

Digital Vaccination certificates cannot be issued to the following people.

- People who do not have their own My Number Card (My Number Card is required for application)
- People who do not have a smartphone (The app supports iOS and Android. Devices that can read my number card are required.)
- People with a maiden name, a different name, or an alias (Because there is a risk that information may be unintentionally passed on to the perpetrator, etc. through automatic delivery.)
- Persons requiring consideration, such as victims of domestic violence (Because there is a possibility of unintentional information being passed on to the perpetrator through automatic issuance.)
- Persons whose vaccination records have not been registered with the VRS (because the VRS cannot issue the vaccination without a record to refer to)
- Persons requesting travel documents other than passports (for those who need a travel certificate, the MRZ of the passport will be read)

Paper-based Vaccination Certificate

How to apply

There are two ways to apply.

You can request either by post or at the counter in Tsukuba City Hall.

1.  Request by post
Please fill in an Application Form for Vaccination Certificate of COVID-19 (Vaccine Passport) and send it to the following address, along with required documents.
Tsukuba City will send your COVID-19 Vaccine Passport by post after confirming the above-mentioned documents.

2.   Request at the counter in the City Hall
Please fill in an Application Form for Vaccination Certificate of COVID-19 (Vaccine Passport) and bring it to the City Hall, along with required documents.
Tsukuba City will send your COVID-19 Vaccine Passport by post after confirming the above-mentioned documents.

Application Form

Required documents

All applicants need to submit the following documents:

1. Application Form (The form is downloadable from the link above.)
2. ID

For international travel or domestic use Passport
For domestic use One of the following cards and documents:
Residence card, special permanent resident certificate, passport, driver's license, Individual Number (My Number) Card and ID card issued by a national or local government authority.

3. Vaccination voucher (with stickers indicating “pre-vaccination screening only (予診のみ)” and Certificate of Vaccination for COVID-19 attached) or Record of Vaccination for COVID-19 (for healthcare workers)

※If you lost your vaccination voucher, please bring your ID which shows the address at the time of being vaccinated can also be accepted.

The following documents are required for some applicants.

4. Your ID that clearly indicates your birth/maiden name, alternative surname, and/or alternative first name (If your passport shows your birth/maiden name, alternative surname, and/or alternative first name.)

The following documents are required for application by power of attorney. 


5. Power of attorney (If someone other than the parent or relative living together is filing an application, power of attorney is required.)

Please note that a family member or relative residing at the same address needs to provide power of attorney, if he/she belongs to a separate household.

6. The ID of a person (parent/guardian) filing an application
If someone (parent/guardian) is requesting a Vaccine Passport on their child’s behalf, an ID of the parent/guardian such as residence card, driver’s license, “My Number Card”, and health insurance card is required.

Important note for required document

If requesting by post, please send a copy of documents indicated #2 to 4 and #6. For passport, please make a copy of a page with photo.

Please note that we cannot issue a Vaccine Passport if your passport has expired.
If you renew your passport and your passport number is changed after your Vaccine Passport is issued, you need to request a Vaccine Passport again. If you are waiting for your passport to be issued, please request a Vaccine Passport after you receive your passport. Foreign nationals with a passport issued by foreign government can also request a Vaccine Passport.
If the parent/guardian is requesting a Vaccine Passport on their child’s behalf, a document that shows the relationship such as a residence certificate and maternal and child health handbook is required. (If the parent/guardian is living with the child and registered in the same household as the child, this document is not required.)

Important points to note:

1.It will be delivered about 6 to 10 working days from the date of application for Vaccination Certificate of COVID-19(Vaccibe Passport) . Please note that it may take longer if there are missing documents.

2. It is free of charge to issue a Vaccine Passport.

3. If there are any incomplete documents, there may be cases where a staff from Tsukuba City contacts you. Please be sure to fill in your contact details in an application form.

Q&A (From the official website of the national government)

How long is a Vaccine Passport valid?
There is no term of validity for Vaccine Passport, because it is to certify the fact that the person is vaccinated. How it is assessed and/or used will depend on each country, etc.

If I received two vaccine doses in different municipalities, can I get certificates for both doses at either one of the municipalities?

No, you will be required to apply for a certificate at each local administration.

Can I get a vaccination certificate for a child who does not have a smartphone, using the same smartphone as the parent?
Yes, it is possible for a person with parental authority as a legal representative to obtain a child's vaccination certificate using their own smartphone. Multiple vaccination information will be stored in the same app.
【Digital Certificate for International travel】
I got married and changed my name. What should I do if my name on my passport is different from my name on My Number card?
The vaccination certificate for international travel will contain your "kanji name” on your My Number Card, and your "romaji name" on your passport.
If the information on your passport and My Number Card are different, please match the names on your passport and My Number Card before you apply.
Is there a limit to the number of copies I can request for Vaccine Passport?
In principle, one copy is issued per application. There is no limit regarding how many times you can apply.
Which countries recognize Vaccine Passport and what kind of mitigation measures will become available if I have a Vaccine Passport?

We are in the process of collecting information about vaccination certificate/passport and relaxation of restrictions in various countries at the moment so that Japan’s Vaccine Passport will be accepted and bring benefits such as shortening quarantine period. In general terms, however, measures such as shortening or waiver of quarantine period at the time of entering a country, and/or waiver of certificate of negative PCR test result for COVID-19 before departure and at the time of arrival, may be considered. Detailed information regarding which countries and regions accept Japan’s Vaccine Passport and what kind of mitigation measures they have will be provided as soon as they become available.
