Vaccination Certificate of COVID-19 (“Vaccine Passport”)

Vaccination Certificate of COVID-19 (“Vaccine Passport”) certifies that you were vaccinated against COVID-19. For the time being, this Vaccine Passport is intended for use by persons travelling abroad, for reasons such as presentation at the time of entering a foreign country, to shorten quarantine time, etc. For this reason, a Vaccine Passport will be issued for only those who plan to travel abroad in the near future.
Please also note that if you would like to issue a Vaccine Passport in Tsukuba City:
・You must be vaccinated using vaccination vouchers issued by Tsukuba City.
・If you moved out of Tsukuba City, your “Vaccination Passport” can be issued in Tsukuba City as long as you were registered as a resident and vaccinated in the City in the past.

That means if you had your first and second doses in different municipalities because you moved houses or other reasons, you need to request both municipalities to issue a vaccine passport.

Note: If you need to confirm your vaccination record or a COVID-19 Vaccination Certificate issued by a municipality for domestic use, because you lost your “Record of Vaccination for COVID-19” or “Certificate of Vaccination for COVID-19 (attached to the vaccination voucher)”, please refer to the website about “COVID-19 Vaccination Certificate (for domestic use)” and request a certificate according to the instruction on the page.

How to apply

Tsukuba City will start accepting requests for a Vaccine Passport on July 26 (Mon), 2021. You can request either by post or at the counter in Tsukuba City Hall.

 

1.  Request by post
Please fill in an Application Form for Vaccination Certificate of COVID-19 (Vaccine Passport) and send it to the following address, along with required documents.
Tsukuba City will send your COVID-19 Vaccine Passport by post after confirming the above-mentioned documents.

2.   Request at the counter in the City Hall
Please fill in an Application Form for Vaccination Certificate of COVID-19 (Vaccine Passport) and bring it to the City Hall, along with required documents.
Tsukuba City will send your COVID-19 Vaccine Passport by post after confirming the above-mentioned documents.

Application Form

Required documents

Vaccination voucher (with stickers indicating “pre-vaccination screening only (予診のみ)”

All applicants need to submit the following documents:

1. Application Form (The form is downloadable from the link above.)
2. Valid passport at the time of traveling abroad (A passport which belongs to a person who were vaccinated and need a certificate.)
3. Vaccination voucher (with stickers indicating “pre-vaccination screening only (予診のみ)” and Certificate of Vaccination for COVID-19 attached )

If you lost your vaccination voucher, please bring your "My Number Card" or other document such as a certificate of residence which clearly shows your "My Number". Other form of ID which shows the address at the time of being vaccinated can also be accepted. For postal application, please send a copy of #2 and 3.

The following documents are required for some applicants.
4. Record of Vaccination for COVID-19 (for healthcare workers).
5. Your ID that clearly indicates your birth/maiden name, alternative surname, and/or alternative first name (If your passport shows your birth/maiden name, alternative surname, and/or alternative first name.)

The following documents are required for application by power of attorney.
6. Power of attorney (If someone other than the parent or relative living together is filing an application power of attorney is required.)

Please note that a family member or relative residing at the same address needs to provide power of attorney, if he/she belongs to a separate household.

7. The ID of a person (parent/guardian) filing an application
If someone (parent/guardian) is requesting a Vaccine Passport on their child’s behalf, an ID of the parent/guardian such as residence card, driver’s license, “My Number Card”, and health insurance card is required.

Please note that we cannot issue a Vaccine Passport if your passport has expired.
If you renew your passport and your passport number is changed after your Vaccine Passport is issued, you need to request a Vaccine Passport again. If you are waiting for your passport to be issued, please request a Vaccine Passport after you receive your passport. Foreign nationals with a passport issued by foreign government can also request a Vaccine Passport.
If the the parent/guardian  of the person vaccinated (who requires a Vaccine Passport)(guardian), or his/her relative living together is requesting a Vaccine Passport on his/her  their child’s behalf, a document that shows the relationship such as a residence certificate and maternal and child health handbook is required. (If the parent/guardian is living with the child and registered in the same household as the child, this document is not required.)

If requesting by post, please send a copy of documents indicated  #2 to 7. For passport, please make a copy of a page with photo.

Important points to note:

1. It will take approximately one week to issue and send a Vaccination Certificate of COVID-19 (Vaccine Passport) from the date of application. Please note that it may take longer if there are missing documents.

2. It is free of charge to issue a Vaccine Passport.

3. If there are any incomplete documents, there may be cases where a staff from Tsukuba City contacts you. Please be sure to fill in your contact details in an application form.

Q&A (From the official website of the national government)

How long is a Vaccine Passport valid?
There is no term of validity for Vaccine Passport, because it is to certify the fact that the person is vaccinated. How it is assessed and/or used will depend on each country, etc.
Which countries recognize Vaccine Passport and what kind of mitigation measures will become available if I have a Vaccine Passport?

We are in the process of collecting information about vaccination certificate/passport and relaxation of restrictions in various countries at the moment so that Japan’s Vaccine Passport will be accepted and bring benefits such as shortening quarantine period. In general terms, however, measures such as shortening or waiver of quarantine period at the time of entering a country, and/or waiver of certificate of negative PCR test result for COVID-19 before departure and at the time of arrival, may be considered. Detailed information regarding which countries and regions accept Japan’s Vaccine Passport and what kind of mitigation measures they have will be provided as soon as they become available. (Detailed information will be available on the official website of the Ministry of Foreign Affairs.)

Is there a limit to the number of copies I can request for Vaccine Passport?
In principle, one copy is issued per application. There is no limit regarding how many times you can apply.
