COVID-19 Vaccination Certificate(for domestic use)

The following documents can be used as proof of being vaccinated.
Please note that these documents are available only for registered residents of Tsukuba City at the time of being vaccinated.
・"Certificate of Vaccination for COVID-19" attached to the vaccination voucher
・"Record of Vaccination for COVID-19" for healthcare workers, etc.

Healthcare workers, etc. who require a  COVID-19 Vaccination Certificate issued by Tsukuba City, as well as persons who lost a Certificate of Vaccination for COVID-19 attached to their vaccination vouchers, are able to request for a COVID-19 Vaccination Certificate from the City.

To request a certificate, you must be a resident of Tsukuba City at the time of being vaccinated.

Note: If you are planning to travel overseas in the near future and need a certificate of vaccination in your country of destination. please refer to the website about COVID-19 Vaccination Certificate (“Vaccine Passport”) and apply for it.

How to apply

Tsukuba City's COVID-19 Vaccination Certificate can be requested either by post or at the counter in the City Hall:

1. Request by post
Please fill in an application form for COVID-19 Vaccination Certificate and send it to the following address, along with copies of your ID and Record (for healthcare workers)/Certificate (attached to the vaccination voucher) of Vaccination for COVID-19.
Tsukuba City will send a COVID-19 Vaccination Certificate by post after confirming the above-mentioned documents.

2. Request at the counter in the City Hall
Please fill in an application form for COVID-19 Vaccination Certificate and bring it to the City Hall, along with your ID and Record (for healthcare workers)/Certificate (attached to the vaccination voucher) of Vaccination for COVID-19.

Application form and power of attorney

Required documents

All applicants need to submit the following documents:
1. Application Form (The form is downloadable from the link above.)
2. ID
3. Record of Vaccination for COVID-19 (if you are a healthcare worker and if you have it)
4. Vaccination voucher (with stickers indicating Certificate of Vaccination for COVID-19 attached) (if you are not a healthcare worker and if you have it)

The following documents are required for application by power of attorney.
5. Power of attorney (If someone other than the parent/guardian or relative living together is filing an application, power of attorney is required.)
The form for power of attorney is downloadable from the link above.

- The ID of a person filing an application is required.

- If applying at the counter, please present the ID. (No need to hand it in.)
Please refer to #3 of Important points to note for details of IDs.
- If a parent/guardian is requesting a certificate on his/her child’s behalf, a document that shows the relationship such as a residence certificate and maternal and child health handbook is required. (This is not required if the parent/guardian lives with his/her child and belongs to the same household.)
- If requesting by post, please send a copy of documents #2 to 4.

Important points to note:

1. It will take approximately one week to issue and send a COVID-19 Vaccination Certificate from the date of application. Please note that it may take longer if there are missing documents such as vaccination voucher or vaccination record mentioned above.

2. It is free of charge to issue a COVID-19 Vaccination Certificate.
3. ID
One of the following cards and documents:
Residence card, special permanent resident certificate, passport, driver's license, Individual Number (My Number) Card and ID card issued by a national or local government authority.


4. There may be cases where the City staff contacts the applicant if the documents are incomplete, so please fill in the contact information.
